Xayum, there is social transition and there is medical transition.  Each have a number of steps to consider, so it’s not clear exactly what you are wanting to do.   I think you should be seeing a therapist to help you with either or both, but I in effect was well down the road to social transition before any medical changes.  In fact that used to be the standard of care to live as the chosen gender for a year before any medical changes.
