Primary Care Providers

We are one of the largest non-profit family planning clinics in Montana, and are a 501(c)(3) organization. About 8% of our budget comes from federal grants and funding.  The remaining funding primarily comes from donations and visit fees. Our fees are based on patient income and are assessed according to ability to pay.  We depend on you to pay for your care or make a donation at the time of your visit so that we can continue to provide you with personal, affordable, and professional service. The CHP-Bozeman medical clinic has been offering primary and preventative medical care to patients of all ages as well as behavioral health for adults since 2001. The clinic is in the heart of Bozeman’s historic downtown with convenient, free street parking and in the CHP-owned lot east of CHP’s building and across Mendenhall Street. Services are provided by experienced family physicians, family nurse practitioners, physician assistants, licensed clinical professional counselors, and licensed clinical social workers. A resource coordinator/certified application counselor also is available to assist with referrals to community resources, assistance with application paperwork, and help with accessing public benefits Open Listing
